Simply fill

this page, or e-mail sales@puppiaus.com
indicating the item name,
product code, color, size
and quantity you wish to
order. We will then follow
up with a phone call or
send you an e-mail to complete
your ordering process and
arrange for delivery.
You can also download our
inquiry form as an Excel
file. If you prefer, you
can fax the form to: 1-201-944-2999
after you print out the
form.
